Information for Submissions from EU/EEA* countries:

The MGZ – Medical Genetics Center accepts test requests for genetic diagnostics via the submission of an E-112/S2 form for legally insured outpatients.

The following conditions apply:

• The cost carrier for the diagnostics is the respective social/health insurance of the patient’s foreign country of residence.
• Approval for the desired diagnostics by the patient's social/health insurance is required (stamp/seal of the insurance provider, date, and signature).

Please note that the form must include the following:

• Clinical indication(s), diagnosis and/or suspected diagnosis, name of the analysis and/or test number (please see website or test request form for more details)
• Duration of diagnostics - a validity period of at least 3 months from the date of sample shipment
• The MGZ - Medical Genetics Center must be named as the service provider.

Diagnostics can only be started after receipt (by post, fax, or e-mail) of the correctly completed and approved E-112/S2 form.

Exception: Prenatal diagnostics will be started immediately, a subsequent submission of the E-112/S2 form is possible.

EEA Countries: Iceland, Liechtenstein, and Norway

Information für Einsendungen aus EU/EWR*-Ländern:

Das MGZ – Medical Genetics Center akzeptiert eine Einsendung mit einem E-112-/S2-Formular (nur gültig für EU-Länder*) für gesetzlich versicherte ambulante Patienten.

Grundsätzlich gilt:

• Kostenträger für die Diagnostik ist die jeweilige Sozial-/Krankenversicherung des Patienten an seinem ausländischen Wohnort
• Genehmigung der Sozial-/Krankenversicherung des Patienten für die gewünschte Diagnostik (Stempel, Datum, Unterschrift)

Bitte beachten Sie, dass das Formular folgendes enthalten muss:

• Angabe der genauen Indikation(en), Diagnose und/oder Verdachtsdiagnose Diagnostik und Verdachtsdiagnose, Name der Analyse und/oder Testnummer (weitere Informationen finden Sie auf der Website oder unser Anforderungsbogen).
• Dauer der Diagnostik: mindestens 3 Monate ab Verschickungsdatum
• Das MGZ - Medical Genetics Center muss als Dienstleister namentlich genannt sein

Erst ab Erhalt (per Post, per Fax, per E-Mail) des korrekt ausgefüllten und genehmigten E-112/S2-Formulars kann die Diagnostik gestartet werden.

Ausnahme: pränatale Diagnostik wird sofort gestartet, eine Nachreichung des E-112/S2-Formulars ist möglich.

*Berechtigte Länder: Alle Länder der Europäischen Union (EU) & des Europäischen Wirtschaftsraums (EWR) + Schweiz
